EARLY INTERVENTION
SERVICE COORDINATION
SERVING CHILDREN
FROM BIRTH TO THREE YEARS |
|
|
The Early Intervention Program is funded and regulated by the New
York State Department of Health and by the New York City Department
of Mental Health, Mental Retardation and Alcoholism Services.
In New York State, EARLY INTERVENTION SERVICES are an
entitlement under the law for all children between birth and three
years of age who are exhibiting developmental delays or who have
conditions that will, in all likelihood, place them at risk for
delayed development.
As of July 1, 1993, the Early Intervention Program was established
by the New York State legislature to provide coordinated and
comprehensive early intervention services to infants and toddlers
with disabilities and their families.

How do
Families Access Services Coordination?
A parent, early intervention (EI) provider or other referral
source may call 311 and ask for “Early Intervention”.
|
|
Once
A Service Coordinator is assigned, this Individual will:
-
Inform the parent of
their rights and responsibilities within the program
-
Assist a family in
choosing an evaluation site
-
Arrange for the
evaluation
-
Arrange an initial
Individual Family Service Plan (IFSP) meeting which will outline the
services needed by the child and his/her family, as well as the
provider of services and place of service provision.
