Supporting People in Crisis
AHRC New York City’s Intensive Case Management / Technical Assistance Program provides crisis intervention and emergency case management to people with suspected or confirmed intellectual and/or developmental disabilities (I/DD) and their caregivers.
The program helps prevent medically unnecessary and extended hospitalizations while improving the functional status and quality of life for all served.
Who is eligible for the program?
- People with a suspected or confirmed I/DD diagnosis who are in crisis or on the verge of crisis.
- Caregivers of those individuals.
What services are provided?
- Assessment to determine the appropriate level of service and support needed for stability in the community.
- Supportive clinical services delivered in-person, via phone, or through teletherapy.
- Case management to stabilize individuals, families, and other caregivers in crisis.
- Linkages to benefits, entitlements, and other supports to assist families and caregivers, particularly those without appropriate habilitative services in place.

- NYC Health + Hospitals Inpatient Units
- Schools
- Care Coordination Organizations (CCOs)
- Other Community Providers
