Supporting Safe Transitions to Community Living
What does this program offer?
AHRC New York City’s Crisis Respite Transition Program (CRTP) is a 5-bed specialized transitional housing program. It supports safe hospital discharges for people who no longer require hospital-based medical or psychiatric care but are not able to return to their place of residence because their families or caregivers are unable to support their needs.
Who is eligible for the program?
To be eligible for CRTP services, individuals must meet all of the following criteria:
- Be ambulatory adults (18 years and older) with a suspected or confirmed I/DD diagnosis.
- Be psychiatrically stable.
- Not require skilled nursing care.
- Not have a substance use disorder.
- Not receive services in a certified OPWDD residential program.
What services are provided?
- Safe and appropriate temporary housing, including 24/7 supervision and direct care.
- Positive and trauma-informed behavior support.
- Needs assessment and development of a support plan.
- Structured activities.
- Supervision with medication administration and medical appointments.
- Linkage with long-term supports and services.
- Transition planning and support to secure permanent community housing.

- NYC Health + Hospitals Emergency Departments (ED, PES, CPEP)
- NYC Health + Hospitals Inpatient Units
