We provide a variety of clinical services and access to outside health and safety resources.
Services can be provided to individuals of all ages residing in all five boroughs.
AHRC NYC provides a wide range of Clinical Services at Article 16 clinics and various sites throughout the five boroughs of New York City. These services are offered to individuals with developmental disabilities, ages 5 through the senior years. Additionally, certain support services can be provided to other family members who do not themselves have a disability.
Most services provided are covered by Medicaid and/or Medicare. If an individual does not have Medicaid or Medicare, we may be able to help to obtain it. If you are a member of an HMO or other managed care plan, you may or may not be eligible to receive certain services. For individuals with no coverage, we offer a sliding scale fee.
Care Coordination
Care Coordination is a new model for coordinating both the healthcare and long-term support service needs of individuals, through a Care Coordination Organization/Health Home (CCO/HH). One of the largest of these new CCO’s, Care Design New York (CDNY) was created by AHRC New York City and other New York City and State I/DD providers to replace MSC services. The Care Manager will work on a care management team that is responsible for coordinating both the healthcare and long-term support service needs of individuals.
Day Services
Day Services supports adults with intellectual and other developmental disabilities, traumatic brain injury, and autism spectrum disorders to live self-determined and meaningful lives in their communities. Using a person-centered framework where people supported define what they want, we support them to contribute, participate, and become valued members of the community.
